If you don't have the plunger cap on, shit can fall inside of the barrel when the plunger is fully depressed, as it is when carried around.

The cap being replaced prevents debris from falling into the inside of your barrel. You know, that part you want sterile.

Yes you can tell. Unused rigs look like the picture shown except there's a 2cc air bubble at the tip. Used syringes are typically pushed all the way to the 0cc mark and/or have obvious signs of blood in them.

Tons of people put the plunger caps back on after use. It keeps the inside of the syringe clean for later use, because let's be honest, IV drug users will typically reuse them until the needle is too dull to function.
